Myasthenia Gravis VA Disability Rating

OVERVIEW
Myasthenia Gravis (MG) is rated under DC 8025 at 38 C.F.R. § 4.124a with a minimum rating of 30%. Higher ratings are assigned for residual disabilities using the appropriate body system codes. Veterans may develop MG from thymoma treated during service, or as an autoimmune condition that onset during or was aggravated by service.
RATING CRITERIA (4 LEVELS)
100% — Severe MG with respiratory compromise or inability to perform ADLs
Myasthenic crisis requiring ventilatory support; OR inability to perform ADLs independently. Rate under appropriate residual codes.
70% — Severe residual disability
Marked weakness affecting multiple muscle groups including respiratory and bulbar muscles with significant occupational and daily living limitation.
50% — Moderate to severe residual disability
Moderate-severe weakness affecting swallowing, speech, or limb function with significant functional limitation.
30% — Minimum rating (any diagnosis)
Minimum 30% evaluation upon diagnosis of Myasthenia Gravis. Per 38 C.F.R. § 4.124a, DC 8025. Mild ptosis, mild limb weakness, well-controlled with medication.
KEY EVIDENCE TO GATHER
-Neurologist evaluation confirming Myasthenia Gravis
-AChR-Ab or anti-MuSK antibody testing
-Repetitive nerve stimulation or single-fiber EMG showing decremental response
-CT chest for thymoma evaluation
-Treatment records (pyridostigmine, IVIG, plasma exchange, immunosuppressants)
-Records of myasthenic crises requiring hospitalization or ventilatory support

C&P EXAM TIPS (4)
1.Minimum 30% upon diagnosis — no veteran with confirmed MG receives less than 30%.
2.Higher ratings assigned for residual disabilities — document all functional limitations.
3.Myasthenic crises requiring hospitalization support higher ratings.
4.If ptosis blocks your visual field, document this — it rates under eye codes separately.

Minimum ratings for residuals under 8000-8025 require ascertainable residuals. Subjective residuals accepted when consistent with the disease and not more likely attributable to other disease.
