Kidney Stones / Nephrolithiasis VA Disability Rating

OVERVIEW
Kidney stones (nephrolithiasis) are rated under DC 7508 (nephrolithiasis) or DC 7509 (hydronephrosis) based on recurrence and complications. Common in veterans due to dehydration during deployments, high-protein military diets, and limited water access in field conditions. The condition is rated based on whether it requires drug therapy, diet therapy, or invasive procedures.
RATING CRITERIA (4 LEVELS)
30% — Recurrent stone formation requiring invasive/noninvasive procedures more than 2 times/year
Under DC 7508, recurrent nephrolithiasis requiring invasive or noninvasive procedures more than two times per year automatically rates at 30%. Rate under DC 7508 (nephrolithiasis/ureterolithiasis/nephrocalcinosis).
30% — Frequent colic with infection and impaired kidney function (DC 7509 hydronephrosis)
Under DC 7509 (hydronephrosis): frequent attacks of colic with infection (pyonephrosis) AND kidney function impaired. Rate kidney stones as hydronephrosis when hydronephrosis is present.
20% — Frequent colic requiring catheter drainage (DC 7509)
Under DC 7509: frequent attacks of colic requiring catheter drainage.
10% — Occasional colic, not infected, not requiring catheter drainage
Under DC 7509: only occasional attacks of colic, not infected and not requiring catheter drainage. Most kidney stone veterans who are not in the recurrent procedure category rate here.
KEY EVIDENCE TO GATHER
-CT scans or ultrasounds showing kidney stones (current and historical)
-Urology records documenting stone composition, treatments, and recurrence pattern
-Service records showing deployment to hot climates with limited hydration
-Emergency room or surgical records for stone passages or procedures (lithotripsy, stenting)
-Documentation of preventive medication (potassium citrate, allopurinol, thiazide diuretics)

DC 7500-7502
Recurrent stones and obstruction can cause progressive kidney damage.
DC 7512
Stones provide surfaces for bacterial colonization, causing recurrent UTIs.
C&P EXAM TIPS (4)
1.Keep records of EVERY stone episode: date, treatment, ER visits, missed work, procedures performed.
2.The key to a 30% rating is documenting that you need drug therapy, diet therapy, AND/OR multiple procedures per year.
3.Link your stones to service by showing deployment to hot climates where hydration was limited.
4.Request stone composition analysis. Certain stone types (uric acid) have clear links to diet and dehydration.

Nephrolithiasis (DC 7508) rates as hydronephrosis (DC 7509) EXCEPT when recurrent stone formation requires invasive or noninvasive procedures more than twice per year — in that case, rate at 30% under DC 7508 directly. Monitor kidney function after stone episodes for possible separate CKD rating.
