Your VR&E counselor will complete and send this form to your school or training facility. Review it to confirm: (1) Your name and SSN are correct, (2) The correct training program and school are listed, (3) The authorized start and end dates match your expectations, (4) The items VA will pay for are listed correctly (tuition, books, tools, supplies, possibly a subsistence allowance), (5) Any special conditions or restrictions are acceptable.

Common mistakes
- ×Not reviewing the form before the school receives it - errors are harder to correct after enrollment begins.
- ×Not understanding that VA pays the school directly for tuition and books; the subsistence allowance goes to you separately.

The school certifying official at your training institution signs the form to confirm enrollment and verifies your attendance monthly. Attendance verification triggers monthly subsistence allowance payments. If you miss class, report it to your VR&E counselor - unexcused absences can affect benefits.
