Section II - Tutoring Details

Name the specific course (required) and describe the academic deficiency. Identify the tutor (name, qualifications), number of hours tutored this month, and the hourly rate. VA pays a maximum of $100/month. The school certifying official must countersign confirming the tutoring is for a required course.
e.g., Calculus I (required for Engineering degree, currently have C-); Tutor: Dr. Jane Jones, PhD Math; 5 hours x $20/hr = $100.
Common mistakes
